This form is a list of possible lab tests that may be ordered for diagnosis of your condition. This list can be copied into Word (edit, select all, copy, paste) and printed.

In general, most specialty labs (e.g. for food allergies, GI assessment, or heavy metals…) have a requisition form that is included in the kit. That requisition form should be filled out and included with the sample.

I do not do blood draws for specialty lab draws. Instead, I write a draw request which you can take to a hospital laboratory near you. Take the kit, the signed requisition for the lab test, and my draw order with you to when you go for the blood draw.

Call the draw station/Lab to make an appointment if necessary.
Make sure you go to the draw station early enough so that the sample can be picked up by FedEx/UPS that day.
Freeze any shipping media if required.

In general: fast after dinner, and go in for blood draw in the AM.
